DEPARTMENT OF HEALTH INFORMATION MANAGEMENT
INTRODUCTION:
Health Record is an essential component in the effective management of patients’ health care. The Health records contains the information needed to plan, provide and evaluate the care provided. It serves as a tool for communicating information to all health care provides who deal with the patients. To fulfill these functions successfully requires not only that the health records met certain minimum standard but a system for making the information contained in the records available when and where needed. Information collated, analyzed and interpreted help in making better and problem driven decisions.
SERVICES RENDERED:
The department has as it major roles:
- Documentation of patient both as new and old.
- Maintain and retention of patients’ health records for future care or research.
- Collate, analysed and interpret data using indicators for qualitative and quantitative management of activities.
- Maintain records of cases seen and treated in the hospital.
- Provide necessary information needed by Management on the day-to-day activities of the Hospital for decision making.
II. OUTPATIENT SERVICES [ 2009 ]
111,710 patients received services on outpatient basis 31,115 new and 80,595 old cases. January, June and October recorded highest attendance of 7,554, 7,498 and 7,516 respectively. The average daily clinic attendance was 428 patients. In departmental attendance, General Outpatient recorded the highest attendance of 33,235, followed by ANC 21,321 and HIV 16,739.
III. OPERATIONS [2009]
1720 operations were performed within the year. April recorded the highest operation of 172 followed by June 168, March and April 163 and 162 respectively.
In department basis, Obstetrics and Gynaecology recoded 58% of the total operations. Obstetrics with 693 and Gynaecology with 304. The next department was General surgery with 243 followed by Paediatric with 151. See table below.
IV. DELIVERIES [2009]
The year recorded 1920 births; Live births of 1908 and still birth of 112. 8 maternal deaths were recorded. One (1) in Labour ward and 7 in post natal ward. The still birth rate is 58 per 1000 live births. There was a remarkable improvement when compared with 2008 which was 70 per 1000 live births. Maternal death dropped by almost 50% from 15 in 2008 to 8 in 2009.
Deliveries by age group shows that ages 25 – 34 recorded 72% of the total deliveries. Age group 25 – 34 had 1386; 15 – 24 had 335; 35 – 44 had 199. Monthly delivery record shows that June, May and April had the highest delivery of 191, 198 and 188 respectively.
V ACCIDENT AND EMERGENCY: [2009]
The Department recorded 5681 attendance; 4624 new and 1057 old. There were some special cases reported in the department within the year. 967 accidents cases were reported; 927 Road Traffic Accidents, 32 domestic accidents and 8 Industrial Accidents. 70 cases of Gunshot were reported; 10 cases of poisoning; 8 cases of food poisoning and 2 cases of C2O. 40 burns, 16 rape cases, 265 assaults, I electrocuted; 3 magic torch/lose of manhood and 36 bites; 11 human bites, 18 dog bites, 6 snake bite and 1 insect bite.
VI SPECIAL PROCEDURES: [2009]
1. AUDIOMETRY: The unit carried out 150 examinations; 94 males and 56 females
2. DIALYSIS: 353 Dialysis were conducted in the year. 155 new cases and 238 old cases.
3. ECG: 620 Electrocardiogram examinations were conducted, 269 males and 351 females.
4. X-RAY: 4,384 examinations were conducted in different parts of body. 2423 males and 1961 females.
